Videocassettes |

Use this form to order required videocassettes for your distance
education courses. The cost for this service is $70 (plus $3.50 sales tax for Maryland
residents). Orders are accepted by mail or fax only. A refund of $40 (plus $2 sales tax for Maryland residents) will be issued when your videocassettes are returned in satisfactory condition, one week after the final exam. Deposits charged to VISA or MasterCard will be refunded to the credit card used; payment by any other method will be refunded by check. If you drop the course before the first day of the semester, you will receive a full refund, less $8 shipping and handling. However, if you drop the course after the first day of the semester, only $40 (plus $2 sales tax for Maryland addresses) will be refunded. A fee of $5.00 will be charged for tapes that are not rewound. Once you have registered for a course that requires a videocassette, print out the form below, complete the form and mail or fax it as soon as possible to:
